Chapter 5.40. CARNIVALS, FESTIVALS AND CIRCUSES |
§ 5.40.030. Permit—Application requirements.
All applications for the operation and maintenance of a carnival, festival or circus shall be made in writing on a form prescribed by the city clerk. The form shall indicate at least the following:
A.
The name of the person, firm or corporation which will operate the carnival, festival or circus;
B.
The location of the principal place of business of such person, firm or corporation;
C.
The names and addresses of the officers of such firm or corporation;
D.
The length of time the carnival, festival or circus is to be operated;
E.
The hours of the day or night during which the carnival, festival or circus is to be operated;
F.
What provisions have been made for sanitary facilities for persons using the premises upon which the carnival, festival or circus is to be operated;
G.
Whether or not a tent or other temporary structure is to be used in connection with the carnival, festival or circus;
H.
The location at which the applicant proposes to operate the carnival, festival or circus, which shall indicate by legal description or other method acceptable to the city clerk the exact land area to be utilized by the applicant;
I.
Whether or not any local group or organization is to be involved in the promotion, sponsorship or operation of the carnival, festival or circus, and if so, the name of such organization and the name, street address, mailing address, and telephone number of the local individual representing such organization;
J.
Whether or not the applicant intends to serve food or beverage for public consumption, and if so, written proof that the applicant has complied with all state laws and regulations relating to the serving of food and beverages to the public for consumption.
